Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting in West Palm Beach, FL. This PRN role offers flexible assignments with competitive compensation and clinical autonomy within a supportive team.

Candidates must hold a Florida PT license and have a degree in Physical Therapy. New grads are welcome to apply and join a compassionate team focused on preserving independence and quality of life for older adults.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR
