Job summary

Sobe Innovative Rehab in Orlando, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) to provide hospice therapy in a home setting. This PRN role offers flexible assignments, treating residents in their homes or in an assisted living environment while maintaining clinical autonomy.

You'll develop individualized treatment plans, collaborate with physicians and families, and document in the EMR. Active Florida PT license required; new graduates welcome.

Qualifications

• Bachelor's, Master's or Doctorate in Physical Therapy from an accredited program.
• Active Florida Physical Therapist (PT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
