Sobe Innovative Rehab in Fort Walton Beach, FL is seeking a licensed Physical Therapist (DPT/PT/RPT) for PRN home-based hospice care. You will evaluate and treat residents in their homes or assisted living settings, develop individualized treatment plans, and collaborate with physicians and families to maintain independence and quality of life.

Qualifications

• Must hold an active Florida PT license.
