Job summary

Continuum Therapy Partners is seeking a licensed Physical Therapist to join our team on a PRN basis in Sheboygan, WI. You will choose when you work while staying connected to experienced therapists who value quality care.

Requirements include a Doctor of Physical Therapy, an active state license, and NPTE. One year of PT experience is preferred, with responsibilities spanning evaluations, treatment planning, documentation, and patient education.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
