Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our team in Port Washington, WI. This PRN role offers the flexibility to build a schedule around your life while remaining part of a supportive clinical team focused on quality patient care.

The ideal candidate holds a Doctor of Physical Therapy (DPT), a current Wisconsin license, and NPTE eligibility. Prior PT experience is preferred, with a commitment to educating patients and families on home exercise programs.

• Doctor of Physical Therapy (DPT) degree is required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE).
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
