Job summary

Qualifications

• Intermediate computer skills are needed for documentation and records.
• Demonstrated organization, decision-making, and problem solving abilities are essential.
• Team members must maintain annual educational requirements for hospital-based certifications (e.g. DNV, CARF or others).

Responsibilities

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement individualized treatment based on evaluation results and functional goals.
• Monitor outcomes and adjust treatment plans; provide status updates to physicians.
• Plan for discharge including home programs, patient/family teaching, and referrals for community services.
• Complete documentation per department policy, including progress notes, conference reports, discharge summaries, billing, and scheduling; apply insurance guidelines for authorization and visits.
• Maintain clean, organized treatment areas and assist with positioning, lifting, and transferring as needed.
• Demonstrate knowledge of modalities and exercise equipment; understand indications/contraindications.
• Participate in performance improvement activities and coverage planning; mentor newer staff.
• Pursue professional growth through continuing education and department inservices.
