Element Health Services is seeking a dedicated Physical Therapist to join our team. In this role, you will make a meaningful impact in patients' lives while working in community and home environments.

You'll evaluate, plan care, and collaborate with an interdisciplinary team. You will educate patients and families, coordinate with physicians, and document progress on the same day of service to support optimal outcomes.

Qualifikationen

• Graduation from an accredited Physical Therapy program.
• Active PT license in Florida.
• Current BLS certification.
• Outpatient/Medicare Part B experience preferred.
• 2–4 years of experience preferred.

Aufgaben

• Perform initial and ongoing patient assessments.
• Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families on therapy programs.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.

Kenntnisse

Active Florida PT license

Current BLS certification

Outpatient/Medicare Part B experience

2–4 years experience
