Job summary

Family Health Centers of San Diego seeks an experienced Physical Therapist to evaluate and treat adults with mobility, motor control, and musculoskeletal disorders. You will collaborate with referral and care teams, provide home program activities, and supervise assistants to ensure safe, high-quality care.

Travel to various clinic sites will be required. The role requires at least 2 years of PT experience, California licensure, and CPR/BLS certification.

Qualifications

• California State licensure as a Physical Therapist.
• Current American Heart Association healthcare provider CPR (BLS) or equivalent.
• Bilingual English–Spanish is preferred.
• Ability and means to travel within San Diego County.

Responsibilities

• Communicates with Referral, Case Management, Care Coordination, and Scheduling staff.
• Provides home program activities for carryover to the home environment.
• Supervises Physical Therapy Assistants as necessary to ensure safe, effective care.
• Plans and implements effective treatment plans within scope of practice.
• Monitors progress and conducts re-evaluations as indicated.
• Provides treatment at multiple clinic sites and during home visits.
