Job summary

Reliant Rehabilitation is seeking a Physical Therapist to provide high-quality therapy services in skilled nursing facilities. You will screen, evaluate, and treat patients, develop treatment plans, and coordinate discharge planning while maintaining professionalism and compliance with regulations.

As part of a therapist-led organization, you’ll work with a team of clinicians and have access to ongoing continuing education and leadership development opportunities.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Valid state physical therapy license required.

Responsibilities

• Provide physical therapy and related services to patients in SNF settings.
• Develop and monitor initial and ongoing treatment plans of care and discharge plans.
• Supervise PTAs and Rehab Techs to ensure safe and effective therapy.
• Maintain compliance with state and federal regulations and Reliant policies.
• Foster positive interactions with facilities, patients, and families.
