Job summary

Outcome HC is seeking a Full-Time Physical Therapist Assistant to join our rehabilitation team at the Country Arch Care Center in New Jersey. The role focuses on helping seniors regain mobility, independence, and quality of life in a skilled nursing facility.

You will implement treatment plans under a licensed Physical Therapist, document progress in EMR, participate in care plan meetings, and work with an interdisciplinary team.

Qualifications

• Active Physical Therapist Assistant license in New Jersey (or eligible to obtain).
• Mentorship and training provided for new grads.
• Experience in SNF/geriatrics a plus.

What You’ll Do

• Implement treatment plans developed by the Physical Therapist to improve residents’ strength, balance, gait, and functional mobility
• Treat residents with a variety of orthopedic, neurological, and post-surgical conditions under the supervision of a licensed Physical Therapist
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Report directly to the Physical Therapist and Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist Assistant license in the state of New Jersey (or eligible to obtain)
• New grads welcome! We provide excellent mentorship and training
• Previous experience in SNF, long-term care, or geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team
