Job summary

Rose Mountain Care Center in New Jersey is seeking a dedicated Full-Time Physical Therapist Assistant to join our rehabilitation team. You will implement treatment plans, assist with mobility goals, and coordinate care with an interdisciplinary team to support residents' independence.

Qualifications

• Active NJ PTA license or eligibility to obtain.

• Strong communication and interpersonal skills.

Responsibilities

• Implement treatment plans to improve residents' mobility, strength, balance and gait.
• Treat residents with orthopedic, neurological and post-surgical conditions under PT supervision.
• Collaborate with physicians, nurses, OTs and SLPs.
• Document patient progress accurately in the EMR.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a collaborative team.
• Report to the Physical Therapist and Director of Rehabilitation.

What You’ll Do

• Implement treatment plans developed by the Physical Therapist to improve residents’ strength, balance, gait, and functional mobility
• Treat residents with a variety of orthopedic, neurological, and post-surgical conditions under the supervision of a licensed Physical Therapist
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Report directly to the Physical Therapist and Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist Assistant license in the state of New Jersey (or eligible to obtain)

• Previous experience in SNF, long-term care, or geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team
