Job summary

Outcome HC is seeking a full-time Physical Therapist to join our rehabilitation team at Rose Mountain Care Center in New Jersey. You will assess residents, design treatment plans, and work with an interdisciplinary team to restore mobility and quality of life for seniors.

The role offers mentorship for new graduates, opportunities to supervise PTAs, and collaboration with physicians, nurses, and therapists.

Qualifications

• Active Physical Therapist license in New Jersey.
• Mentorship/training provided for new grads.
• Experience in SNF, long-term care, or geriatrics is a plus.

Responsibilities

• Evaluate and treat residents in subacute and long-term care with orthopedic, neurological, and post-surgical conditions.
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility.
• Collaborate with physicians, nurses, occupational and speech therapists.
• Document patient progress accurately in the EMR in a timely manner.
• Participate in care plan meetings and family conferences.
• Supervise Physical Therapist Assistants (PTAs) as needed and report to the Director of Rehabilitation.

What You'll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We're Looking For

• Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
