Job summary

Outcome HC is seeking a Full-Time Physical Therapist to join our rehabilitation team at Shore Pointe Care Center in New Jersey. You will evaluate and treat residents in subacute and long-term care, focusing on mobility, balance, and functional independence.

This role emphasizes collaboration with physicians, nurses, OT and SLP, documentation in EMR, and supervision of PTAs. New grads welcome with mentorship.

Qualifications

• Active Physical Therapist license in New Jersey or eligible to obtain.
• Mentorship and training provided for new grads.
• Experience in SNF, long-term care, or geriatrics is a plus.

Responsibilities

• Evaluate and treat residents in subacute and long-term care with orthopedic, neurological, and post-surgical conditions.
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility.
• Collaborate with physicians, nurses, PTAs, OTs, and SLPs.
• Document progress in EMR and participate in care plan meetings and family conferences.
• Supervise PTAs as needed; report to the Director of Rehabilitation.

What You'll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We're Looking For

• Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
