Job summary

Outcome HC is seeking a Full-Time Physical Therapist Assistant for our skilled nursing facility in New Jersey. You will work closely with a multidisciplinary rehabilitation team at the Runnells Center for Rehabilitation and Healthcare to help residents regain mobility and independence.

You will implement PT plans, document progress in the EMR, and participate in care plan meetings, with mentorship provided.

Qualifications

• Active Physical Therapist Assistant license in New Jersey or eligible to obtain.
• New grads welcome with mentorship and training provided.
• Experience in SNF, long-term care, or geriatrics is a plus.

Responsibilities

• Implement treatment plans developed by the Physical Therapist to improve residents’ strength, balance, gait, and mobility.
• Treat residents with orthopedic, neurological, and post-surgical conditions under supervision of a licensed Physical Therapist.
• Collaborate with physicians, nurses, occupational and speech therapists.
• Document patient progress accurately in the EMR system.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a supportive team environment.
• Report to the Physical Therapist and Director of Rehabilitation.

What You’ll Do

• Implement treatment plans developed by the Physical Therapist to improve residents’ strength, balance, gait, and functional mobility
• Treat residents with a variety of orthopedic, neurological, and post-surgical conditions under the supervision of a licensed Physical Therapist
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Report directly to the Physical Therapist and Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist Assistant license in the state of New Jersey (or eligible to obtain)
