Job summary

Outcome HC is seeking a dedicated Full-Time Physical Therapist for a Skilled Nursing Facility in New Jersey. You’ll evaluate residents, design treatment plans to improve mobility, and work with an interdisciplinary team to restore function and quality of life.

New grads are welcome and will receive mentorship and training; SNF/geriatrics experience is a plus. You will document progress in the EMR and participate in care plan meetings to support residents and families.

Qualifications

• Active Physical Therapist license in New Jersey (or eligible to obtain).
• New grads welcome with mentorship provided.
• Experience in SNF, long-term care or geriatrics is a plus.

Responsibilities

• Evaluate and treat residents in subacute and long term care settings with orthopedic, neurological, and post-surgical conditions.
• Develop personalized treatment plans to improve strength, balance, gait and functional mobility.
• Collaborate with physicians, nurses, OT and SLP for comprehensive care.
• Document patient progress accurately and timely in the EMR.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a supportive team environment.
• Supervise PTAs as needed.
• Report directly to the Director of Rehabilitation.

What You’ll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
