Interim HealthCare of Madison is hiring a Home Health Physical Therapist Assistant in Janesville, WI and surrounding areas. In this role, you will deliver home-based physical therapy to patients, assess mobility, and help restore function in the patient’s home environment.

Qualifications

• Graduate of an accredited PTA program and Wisconsin PTA license.
• Minimum of 1 year PTA experience, preferably in home health.
• CPR certification is required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgement and compassionate patient care.

Responsibilities

• Provide home-based physical therapy to patients with mobility loss.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) to plan care.
• Observe movements, assess progress, and modify care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families on the care plan and safe self-care.
