Job summary

Qualifications

• Active Physical Therapist Assistant license in New Jersey (or eligible).
• New grads welcome with mentorship and training.
• Experience in SNF, long-term care, or geriatrics is a plus.

Responsibilities

• Implement treatment plans developed by the Physical Therapist to improve residents' strength, balance, gait and functional mobility.
• Treat residents with orthopedic, neurological, and post-surgical conditions under supervision of a licensed Physical Therapist.
• Collaborate with physicians, nurses, occupational therapists, and speech therapists.
• Document patient progress accurately in the EMR in a timely manner.
• Participate in care plan meetings and family conferences.
• Provide compassionate, patient-centered care in a supportive team environment.
• Report directly to the Physical Therapist and Director of Rehabilitation.

What You’ll Do

• Implement treatment plans developed by the Physical Therapist to improve residents’ strength, balance, gait, and functional mobility
• Treat residents with a variety of orthopedic, neurological, and post-surgical conditions under the supervision of a licensed Physical Therapist
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Report directly to the Physical Therapist and Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist Assistant license in the state of New Jersey (or eligible to obtain)
