Job summary

Interim HealthCare of the Twin Cities is seeking a Licensed Home Care Physical Therapist Assistant (PTA) for full-time field work serving patients in their homes. Office is in Roseville, MN with territory in the southwest metro.

Qualifications

• Minnesota PTA license active and in good standing.
• Two years PTA experience.

Responsibilities

• Provide services for treating impairments under the direction of a licensed physical therapist.
• Participate in ongoing interdisciplinary assessment of the patient.
• Deliver services as ordered in the plan of care by the physician.
• Complete timely, legible documentation on treatment and patient progress.
• Counsel and educate patient and family to promote progress toward goals.

Hours

Full-time | Monday - Friday | 8:00am - 5:00pm

Territory

This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position would be the southwest metro.

Pay and Benefits

• Hourly Pay Range: $31.40 - $37.44

A few must-haves for this position

• Currently licensedas a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
• Two (2) years of experience as a PTA.
• Able to travel within geographic area serviced by the office.Valid driver'slicense, reliable transportation, and proof of auto insurance.
• CPR/BLS Certification

Functions of what you would do as a Licensed Physical Therapist Assistant

• Provide services for treating impairments, functional limitations, disabilities or changes in physical function under the direction and supervision of a qualified physical therapist.
• Participates in ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Completes and submits timely, legible, accurate, relevant and sufficient documentation addressing the treatment and interventions provided, the patient’s response to the treatment and interventions, and the patient’s progress toward goals in accordance with professional standards of practice, policy and procedures, and payer requirements.
• Counsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
