Job summary

Interim HealthCare in Rochester, MN is seeking a part-time Home Health Physical Therapist Assistant (PTA) to provide home-based therapy and support patients in regaining mobility and strength.

You will work within a home health team, following a patient-centered plan of care, and adapt treatments based on progress and goals. This role emphasizes balance between professional care and family education in a community setting.

Qualifications

• Graduate of an accredited Physical Therapist Program.
• Active Minnesota PTA license.
• CPR certification.
• Minimum 1 year PTA experience, preferably in home healthcare.
• Knowledge of state and federal home health regulations.
• Good clinical judgement and strong interpersonal skills.

Responsibilities

• Provide home-based physical therapy to patients with mobility loss from illness or injury.
• Collaborate with a home health team to meet the patient’s plan of care and goals.
• Observe movements, assess progress, document outcomes, and adjust the care plan as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain.
• Educate patients on plan of care, goals, exercises and self-care activities.
• Coach family members on the plan of care and safe assistance with exercises.

As a Home Health Physical Therapist Assistant, here’s a big-picture view of whatyou’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapist Assistants:

• Graduate of an accredited Physical Therapist Program and active PTA license in Minnesota
• Minimum of 1years of physical therapy assistant experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
