Job summary

Interim HealthCare of the Twin Cities is seeking a Licensed Home Care Physical Therapist Assistant (PTA) to support patients in their homes, with an office in Roseville, MN. The role offers full-time hours, Monday–Friday, 8:00am–5:00pm, serving the southwest metro area.

The ideal candidate holds an active Minnesota PTA license, has at least two years of PTA experience, and can travel within the service area. CPR/BLS certification and a valid driver’s license are required.

Qualifications

• Currently licensed as a Physical Therapist Assistant in Minnesota with active license.
• Two years of PTA experience required.
• Valid driver's license and reliable transportation; proof of auto insurance.
• CPR/BLS certification and compliance.

Responsibilities

• Provide services for treating impairments, functional limitations, disabilities or changes in physical function under the direction and supervision of a qualified physical therapist.
• Participates in ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Completes and submits timely, legible, accurate documentation addressing treatment, interventions, patient progress toward goals, and payer requirements.
• Counsels and educates the patient and family, and other caregivers to promote progress toward mutually established healthcare goals.

Territory

This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position would be the southwest metro.

A few must-haves for this position

• Currently licensedas a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
• Two (2) years of experience as a PTA.
• Able to travel within geographic area serviced by the office.Valid driver'slicense, reliable transportation, and proof of auto insurance.
