Job summary

AccentCare is seeking a Physical Therapist Assistant (PTA) for Home Health in Burnsville, MN. You will work under a licensed Physical Therapist to deliver patient care, educate families, and document treatment plans.

Schedule is Mon-Fri, 8am-5pm, with coverage in Bloomington and surrounding area. The ideal candidate has an associate degree in Physical Therapy, MN license, CPR, and the ability to travel to multiple locations.

Qualifications

• Associates degree required.
• Licensed to practice in Minnesota.
• CPR certification required.
• Must possess and maintain a valid driver's license to travel to all locations.
• Ability to educate patients and families.

Responsibilities

• Provide patient care and document treatment per physician orders, agency policy and patient needs.
• Document interventions, approaches, strategies, materials and establish treatment priorities.
• Provide organized and efficient patient treatment including advising, educating and counseling the patient.
• Present pertinent patient information to supervising therapist and keep team informed of patient status.
• Provide skilled treatment within agency and payer parameters.
• Seek and accept team assistance in planning and intervention.
• Document and submit daily notes; assist with required forms to prevent interruptions in care.
• Meet productivity standards and use work hours efficiently.
• Attend and participate in agency meetings and committees.
• Negotiate patient participation in treatment and schedules.

Paid on a Pay Per Point basis. The compensation reflected on this posting is an estimate of annual compensation for full time status.

What You Need to Know

The Physical Therapy Assistant, under the supervision of a licensed Physical Therapist, is responsible for providing treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. The Physical Therapist Assistant manages a caseload in conjunction with supervising therapist and in accordance with agency productivity standards, attends and participates in patient and other conferences and agency in-services as assigned.

• Associates degree from an accredited school of Physical Therapy
• Have a valid license, registration, or permit to practice in state of agency operation
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
• Be able to implement care plans and communicate any findings or changes in regards to patients back to the Physical Therapist and physician
• Ability to educate patients and their families in basic home safety and medical equipment safety

Responsibilities

As a Physical Therapy Assistant, you will:

• Accurately document, complete and submit daily notes, assist with other required forms in a timely manner to prevent interruptions in patient care.
• Meet agency productivity standards and use work hours efficiently.
• Attend and participate in agency meetings and committees as assigned.
• Appropriately negotiate the patient\'s participation in treatment, treatment times and treatment plans.
