Job summary

Carecore at Willowood is seeking a compassionate PT Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. You will support residents in improving mobility, strength, and independence under the supervision of a Physical Therapist. New graduates are welcome to apply.

Qualifications

Key Responsibilities

• Provide skilled physical therapy treatments under the supervision of a Physical Therapist
• Implement individualized plans of care to reduce pain and improve function
• Monitor and document resident progress, including strength, mobility, and motor performance
• Communicate changes in resident condition to the Physical Therapist and care team
• Educate residents, families, and caregivers
• Collaborate with the interdisciplinary care team
• Maintain accurate, timely, and compliant documentation
• Follow all professional, legal, and regulatory standards

Qualifications

• Active Physical Therapist Assistant state license in good standing
• Skilled nursing or long-term care experience preferred
• Strong clinical, communication, and organizational skills
• Knowledge of the Net Health Electronic Documentation System preferred
