Job summary

CareCore at Willowood in Brunswick, OH, seeks a PRN Physical Therapist to join the in-house rehabilitation team. The role offers flexible scheduling in a skilled nursing and long-term care setting.

You will perform evaluations, develop treatment plans, and deliver evidence-based therapy to improve mobility, strength, balance, and independence, while collaborating with physicians, nurses, residents, and families.

Qualifications

• Current Ohio Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Skilled nursing or long-term care experience preferred, but not required.
• Excellent communication, documentation, and clinical decision-making skills.
• Experience with Net Health and PointClickCare (PCC) is preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide evidence-based therapy services to improve mobility, strength, balance, and independence.
• Collaborate with physicians, nursing staff, therapy team, residents, and families.
• Complete accurate and timely documentation using Net Health.
• Utilize PCC as part of the interdisciplinary care process.
• Maintain compliance with federal, state, and company regulations.

CareCore at Willowood is seeking a compassionate and dedicated PRN Physical Therapist (PT) to join our exceptional in-house rehabilitation team. Position Highlights

Responsibilities

• Perform comprehensive physical therapy evaluations

• Collaborate with physicians, nursing staff, therapy team members, residents, and families

• Complete accurate and timely documentation using Net Health
