Job summary

Willow Valley Center for Nursing & Rehabilitation is seeking a qualified Physical Therapist Assistant (PTA) for flexible PRN hours in a skilled nursing setting. You will work with a licensed Physical Therapist to deliver resident-centered rehab services focused on improving function and safety.

The position emphasizes documentation, collaboration with an interdisciplinary team, and opportunities to assist with discharge planning and home exercise instruction.

Qualifications

  • Graduate of an accredited Physical Therapist Assistant program.
  • Current and valid PTA license in the state of practice.
  • Previous experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
  • Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds when necessary.
  • Strong communication, organization, and teamwork skills.
  • Compassionate, resident-focused approach with a commitment to delivering exceptional care.

Responsibilities

  • Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
  • Assist residents with improving mobility, balance, strength, endurance, and overall functional independence.
  • Implement individualized treatment plans designed to support resident goals and maximize outcomes.
  • Accurately document therapy sessions, resident progress, and daily treatment notes in accordance with facility policies and regulatory requirements.
  • Monitor resident response to treatment and communicate progress, concerns, and recommendations to the supervising Physical Therapist.
  • Assist with discharge planning and educate residents and families on home exercise programs, safety techniques, and continued care strategies.
  • Collaborate with nursing staff, physicians, therapists, and interdisciplinary team members to provide coordinated, comprehensive resident care.
  • Participate in care conferences, therapy meetings, and quality improvement initiatives.
  • Support department operations and provide coverage at nearby affiliated facilities as needed.

As a PTA at Willow Valley, you will work alongside a licensed Physical Therapist and interdisciplinary care team to provide high-quality, resident-centered rehabilitation services focused on improving function, safety, and quality of life.
