Continuum Therapy Partners is seeking a qualified Physical Therapist Assistant to join our PRN team serving Sauk County Health Care Center in Reedsburg, Wisconsin. The role focuses on delivering high-quality care under the supervision of a Physical Therapist and developing individualized treatment plans.

Ideal candidates hold an Associate Degree in Physical Therapist Assistant, current PTA license, and preferably one year of prior PTA experience.

Qualifications

• Associate degree required.
• Current, unencumbered PTA license in state.
• 1 year PTA experience preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
