Summary
The Home Health Physical Therapist Assistant (PTA) performs treatment, provides exercise instruction, and patient education aimed at improving the patient's well-being while following the established plan of care.
Responsibilities
Under the supervision of a Physical Therapist, the PTA is responsible for following all state-specific laws governing physical therapy in home care, adhering strictly to the treatment plan defined by the Physical Therapist, and performing only those procedures they are qualified and trained to perform. The PTA should make recommendations for changes in the plan of care to the supervising PT.
Following the plan of care, the PTA instructs and aids patients in evidence-based treatment within their scope of practice and according to state laws.
The PTA observes, records, and reports to the supervising PT, nurse supervisor, and/or physician the patient's response to treatment and any changes in the patient's condition. They coordinate care with other healthcare team members as appropriate.
The PTA instructs patients, families, caregivers, and other healthcare team members in therapy-related areas within their scope of practice.
The PTA visits patients within the entire service area of their agency and nearby LHC agencies as needed.
Complete a progress note for each visit according to the Plan of Treatment and submit progress notes per LHC policy.
Schedule flexibility is required to provide care as needed, including participation in weekend rotations and potential holiday availability to meet patient demands.
The PTA participates in staff conferences and committees as requested and adheres to LHC Therapy Standards of Practice.
All other duties as assigned.
