Job summary

South Creek Post Acute is seeking a Licensed Physical Therapist Assistant (PTA) in Washington for a part-time role. Under a licensed PT, you’ll provide hands-on care, implement treatment plans, and help residents regain mobility.

You’ll document progress, assist in team meetings, and educate residents and families on safe mobility and exercises. The position emphasizes collaboration, quality care, and a positive culture within a skilled nursing setting.

Qualifications

• Licensed Physical Therapist Assistant in Washington State.
• Strong communication and teamwork skills.
• Experience in skilled nursing or post-acute rehab preferred.
• Familiarity with Casamba or PointClickCare is a plus.

Responsibilities

• Deliver therapy services per the Physical Therapist's care plan.
• Document daily treatments and weekly progress notes on time.
• Support restorative nursing and participate in team meetings.
• Educate residents and families in safe mobility and exercises.
• Maintain treatment areas and equipment in good order.
• Communicate progress and needs with the care team.
