Job summary

Elara Caring is seeking a dedicated Physical Therapist Assistant to support home-based therapy in Gary, Indiana. You will deliver plan-based PT services, help patients reach mobility goals, and collaborate closely with supervising therapists.

The role requires a valid PTA license, a graduate PTA program, and the ability to lift up to 50 pounds. Travel between patient homes is expected; a valid driver's license or reliable transportation is necessary.

Qualifications

• Current, unrestricted PTA license valid for the state of work.
• Graduate of an accredited physical therapy assistant program approved by the American Therapy Association
• 6+ months experience as a Physical Therapist Assistant is preferred
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

Responsibilities

• Provide physical therapy services according to the treatment plan established by the Physical Therapist.
• Help patients work toward therapy goals that improve mobility, function, and independence.
• Monitor patient progress and promptly report changes in condition or concerns to the supervising Physical Therapist.
• Educate patients and caregivers on home therapy programs and techniques to support continued progress.
• Teach patients and families how to safely use assistive devices, including canes, walkers, wheelchairs, prosthetics, and orthotics.
• Deliver safe, high-quality therapy services in accordance with physician orders, clinical standards, and state practice guidelines
