Job summary

Harmony Health and Rehab is seeking a dedicated Physical Therapist Assistant to support a licensed Physical Therapist in a 120-bed skilled nursing environment. The PTA will help residents improve mobility, reduce pain, and regain independence through therapeutic exercises and mobility training.

You will work closely with the PT, families, and the care team to implement treatment plans, monitor progress, and ensure high-quality, compliant care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current and valid licensure/certification as a PTA.
• CPR certification required (or must obtain upon hire).
• Experience in a skilled nursing or long-term care setting preferred.
• Strong clinical, communication, and teamwork skills.

Responsibilities

• Carry out treatment plans designed by the Physical Therapist, including therapeutic exercises, gait training, balance activities, and functional mobility tasks.
• Assist residents in improving strength, range of motion, and overall physical endurance.
• Monitor and document residents’ treatments, progress, and responses in accordance with facility and regulatory standards.
• Communicate effectively with the supervising PT regarding residents’ status, changes, and potential adjustments to care plans.
• Provide education to residents, families, and caregivers on exercise programs, mobility techniques, and safety measures.
• Adhere to facility policies, professional practice standards, and regulatory requirements, including documentation and infection control.
