Aventura at Carriage Inn and Aventura at Shiloh Springs in Dayton & Trotwood, OH, are seeking a Full-Time Physical Therapist to join our rehab team. This split role lets you work across two SNF/LTC facilities, delivering patient-centered therapy and building lasting relationships with residents.

Ohio-licensed PTs, with SNF/LTC experience preferred (new grads welcome), will collaborate with an interdisciplinary team, document in Net Health and PCC, and help residents improve mobility and

Responsibilities

• Evaluate and treat residents in a SNF/LTC setting.
• Develop and implement individualized therapy treatment plans.
• Help residents improve mobility, strength, balance, and overall function.
• Collaborate with nursing, therapy, and interdisciplinary teams.
• Complete documentation using Net Health and PointClickCare (PCC).
• Maintain compliance with facility and regulatory standards.

Setting: Skilled Nursing / Long-Term Care (SNF/LTC)
