Job summary

Aventura at Shiloh Springs in Trotwood, OH is seeking a PRN Physical Therapist to join our in-house rehabilitation team in a SNF/LTC setting. This flexible PRN opportunity emphasizes teamwork and resident-focused care.

Qualified candidates are graduates of an accredited PT program with an active Ohio license. Experience with Net Health and PCC is a plus; strong communication and collaboration skills are essential for this role.

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license in good standing.
• SNF/LTC experience preferred.
• Experience with Net Health and PointClickCare (PCC) preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans of care.
• Provide skilled PT services focused on mobility, strength, balance, safety, and functional independence.
• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team.
• Provide resident and family education as appropriate.
• Complete timely and accurate documentation.
• Support safe discharge planning and continuity of care.

What You'll Do
