Job summary

Aventura Health Group in Dayton, OH is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team at Carriage Inn. You will evaluate residents, develop plans of care, and provide skilled PT services to improve strength, mobility, balance, and independence.

Collaborate with PTAs, nursing, physicians, and the interdisciplinary team while ensuring timely documentation and compliance with state and federal standards.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license in good standing.
• SNF/LTC experience preferred; Net Health and PCC experience preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans of care.
• Provide skilled PT services to improve strength, mobility, balance, and functional independence.
• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team.
• Complete timely and accurate documentation.
• Participate in discharge planning and resident/family education.
• Maintain compliance with facility, state, federal, and professional standards.
