Job summary

Traditional Home Health & Hospice in Peckville, PA, is seeking a Physical Therapist Assistant to provide home-based therapy to patients in their residences. The role is full-time, with hours 8:30am–5:00pm, Monday through Friday, serving Peckville and surrounding areas.

The PTA will implement care plans, educate patients, and document progress, while coordinating with the clinical team to ensure achievement of goals.

Qualifications

• Current, valid Pennsylvania PTA license and CPR certification.
• Valid driver’s license and ability to travel to case assignments.
• Comfortable working in home setting.
• Ability to work independently with minimal supervision and manage time.
• Strong interpersonal and communication skills.

Responsibilities

• Provide therapeutic home physical therapy including strengthening, ROM, balance, gait, and transfer training.
• Instruct patients on home exercises, home safety, incision care, and medications to achieve goals.
• Maintain communication with ordering providers and clinical team to monitor progress.
• Participate in quality improvement activities as assigned.
• Timely and accurate documentation of all patient assessments and interactions (EMR).

Shift(s):

Full-time positions available. Monday through Friday 8:30am to 5:00pm

Our office is located in Dunmore, PA. At Traditional, we do our best to keep each PTA's case load within a sensible geographic territory.

Duties and Responsibilities include, but not limited to the following:

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
• Participate in quality improvement activities as assigned
• Timely and accurately document (EMR) all patient assessments, interactions, etc

Requirements

• Must possess a current, valid Pennsylvania PTA license and have current CPR certification
• Valid driver's license and ability to travel to case assignments
• Must be comfortable working in home setting
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal and communication skills
