Job summary

Traditional Home Health & Hospice is seeking a Physical Therapist Assistant to provide home-based physical therapy across Moosic, PA and surrounding areas. Positions are full-time or part-time with daytime hours; case loads are managed within a sensible geographic territory.

Responsibilities include delivering care per the PT plan, educating patients on exercises and safety, and coordinating with the clinical team. A PA PTA license, CPR, and a valid driver’s license are required.

Qualifications

• Must hold current Pennsylvania PTA license and CPR certification.
• Valid driver's license and ability to travel to case assignments.
• Comfortable working in a home setting and managing time independently.
• Strong interpersonal and communication skills are essential.

Responsibilities

• Provide home physical therapy per established care plan, including strengthening, ROM, balance and gait training.
• Instruct patients on home exercise, safety, incision care, and medications to meet goals.
• Maintain communication with ordering providers, clinical team, and primary PT to track progress.
• Participate in quality improvement activities as assigned.
• Timely and accurate documentation of patient assessments and interactions in the EMR.

Job Title:

Physical Therapist Assistant

Job Location:

Moosic, PA and surrounding areas

Shift(s):

Full-time/Part-time positions available. Monday through Friday 8:30am to 5:00pm

Our Physical Therapist Assistants see patients in the most ideal setting - the patient's home!

Our office is located in Dunmore, PA. At Traditional, we do our best to keep each PTA's case load within a sensible geographic territory.

Duties and Responsibilities include, but not limited to the following:

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
• Participate in quality improvement activities as assigned
• Timely and accurately document (EMR) all patient assessments, interactions, etc

Requirements

• Must possess a current, valid Pennsylvania PTA license and have current CPR certification
• Valid driver's license and ability to travel to case assignments
• Must be comfortable working in home setting
• Ability to work independently with minimal supervision and manage time effectivelyStrong interpersonal and communication skills
