Job summary

Traditional Home Health & Hospice in Moosic, PA, is seeking a Licensed PTA to provide in-home physical therapy services in patients' homes across the Lackawanna area. Full-time/Part-time positions with daytime hours are available, and staff operate mainly in patients' residences with caseloads managed regionally.

Applicants must hold an active PA PTA license and CPR certification, have a valid driver’s license, and be able to travel.

Qualifications

• Must possess a current Pennsylvania PTA license.
• CPR certification required.
• Valid driver's license and ability to travel to case assignments.
• Comfortable working in home setting.
• Ability to work independently with minimal supervision and manage time effectively.
• Strong interpersonal and communication skills.

Responsibilities

• Provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercises.
• Instruct patients on home exercise, home safety, incision care, and medications to achieve goals.
• Maintain communication with ordering providers and the clinical team to promote progress toward goals.
• Participate in quality improvement activities as assigned.
• Timely and accurate documentation (EMR) of all patient interactions.

Our Physical Therapist Assistants see patients in the most ideal setting - the patient's home!

Shift(s)

Full-time/Part-time positions available. Monday through Friday 8:30am to 5:00pm

Our office is located in Dunmore, PA. At Traditional, we do our best to keep each PTA's case load within a sensible geographic territory.

Duties and Responsibilities include, but not limited to the following:

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
• Participate in quality improvement activities as assigned
• Timely and accurately document (EMR) all patient assessments, interactions, etc
