Job summary

Traditional Home Health & Hospice in Peckville, PA is seeking a dedicated Physical Therapist Assistant to provide in-home physical therapy under the supervision of a PT. This full-time role serves patients in the home setting, with a Monday–Friday 8:30am–5:00pm schedule and a Dunmore, PA-based office.

Candidates must hold a Pennsylvania PTA license and CPR certification, possess a valid driver's license, and be comfortable traveling to patient homes.

Qualifications

• Must possess a current, valid Pennsylvania PTA license.
• CPR certification required.
• Must have a valid driver's license and ability to travel to case assignments.
• Comfortable working in home setting and delivering patient education.

Responsibilities

• Provide therapeutic home PT including strengthening, ROM, balance, gait and transfer training.
• Instruct patients on home exercise, safety, incision care, and medications to meet goals.
• Maintain communication with providers and clinical team to track progress and adjust plans.
• Participate in quality improvement activities as assigned.
• Timely and accurate EMR documentation of assessments and interactions.

Job Title: Physical Therapist Assistant
Job Location: Peckville, Pa and surrounding areas
Our Physical Therapist Assistants see patients in the most ideal setting - the patient's home!

Shift(s): Full-time positions available. Monday through Friday 8:30am to 5:00pm
Our office is located in Dunmore, PA. At Traditional, we do our best to keep each PTA's case load within a sensible geographic territory.

Duties and Responsibilities include, but not limited to the following:

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
• Participate in quality improvement activities as assigned
• Timely and accurately document (EMR) all patient assessments, interactions, etc

Requirements:

• Must possess a current, valid Pennsylvania PTA license and have current CPR certification
• Valid driver's license and ability to travel to case assignments
• Must be comfortable working in home setting
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal and communication skills
