Job summary

Consonus Healthcare is seeking a Physical Therapist Assistant to join our Nehalem Valley Care Center team. You will deliver patient-centered therapy to seniors, helping them achieve goals and improve quality of life during times of greatest need.

You will develop personalized care plans, apply evidence-based clinical programs, and collaborate with a dedicated team to maximize function across physical, emotional, social, and cognitive domains for patients and their families.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current state licensure or in the process of obtaining.
• CPR Certification (state/location dependent).
• National Provider Identifier (NPI) required upon hire.
• Excellent organizational skills to facilitate program development.
