Job summary

Consonus Healthcare is seeking a Physical Therapist to deliver person-centered rehab services at Clatsop Care Center in Oregon. You will develop personalized care plans, apply evidence-based programs, and work to maximize patients’ function across physical, emotional, social, and cognitive domains for patients and families.

Qualifications

• Graduate of an accredited physical therapy program.
• Current state licensure -or- in the process of obtaining licensure.
• CPR Certification (requirement varies by state/location).
• National Provider Identifier (NPI) required upon hire.
• Excellent organizational skills to facilitate program development.
• New grads encouraged to apply and grow with us.
