Job summary

H2 Health is seeking a Physical Therapist Assistant to provide 1:1 patient care within luxury Senior Living communities in Naples, Florida. Join a clinician-led network with growth opportunities and a flexible schedule that supports both local placement and travel roles.

You will work with an interdisciplinary team to deliver high quality outpatient rehab services, maintain accurate documentation, and contribute to improving patient outcomes.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active or eligible state license to practice as PTA.
• Prior experience is preferred, but new graduates are welcome.

Responsibilities

• Provide 1:1 physical therapy to residents in Naples, FL.
• Collaborate with interdisciplinary care team to optimize patient outcomes.
• Follow care plans, document progress, and adjust treatment as needed.
