Job summary

Traditional Home Health & Hospice in Moosic, PA seeks a Physical Therapist Assistant to provide home health therapies in Moosic and surrounding areas. Work includes Monday through Friday 8:30am to 5:00pm with full-time/part-time options, focusing on continuing patient care and progress.

You will deliver PT modalities and teach patients home exercise programs. Required PA PTA license and CPR certification, valid driver’s license, ability to travel, and comfort with home-based settings.

Qualifications

Must possess a current, valid Pennsylvania PTA license
CPR certification required
Valid driver’s license and ability to travel to case assignments
Comfortable working in home setting
Ability to work independently with minimal supervision and manage time effectively
Strong interpersonal and communication skills

Responsibilities

Provide therapeutic home physical therapy under PT Care Plan, including strengthening, ROM, balance, gait, and transfer training
Instruct patients on home exercise, safety, incision care, and medications to achieve goals
Maintain communication with ordering providers and clinical team to promote progress toward goals
Participate in quality improvement activities as assigned
Timely and accurate documentation of patient assessments and interactions in EMR
