Job Overview

The role may also involve helping patients manage pain, meet fitness goals, or prevent injury through methods like heat or ice therapy, stretching, massages, and other activities.

Hours

Full-Time, Days 8-4:30 with required weekend rotation and holidays

Minimum Qualifications
  • Associate’s Degree in Physical Therapy.
  • Physical Therapy Assistant license.
  • Basic Life Support (BLS) certification.
  • One (1) year of related experience.
  • A current driver’s license may be required depending on the role.
Preferred Qualifications
  • Three (3) years of community health experience.
Duties and Responsibilities

The duties listed below are intended to describe the general nature of work and are not all-inclusive. Other duties may be assigned.

  • Providing care and services as planned, delegated, and supervised by the Physical Therapist, in accordance with the plan of care.
  • Documenting services in compliance with agency policies and professional standards.
  • Preparing visit notes that include therapy provided, patient response, teaching performed, objective data, and communication with the licensed therapist regarding patient condition and plan adjustments.
  • Participating in in-service education, staff meetings, record reviews, and policy reviews when directed.
  • Planning sessions promptly after notification at times beneficial to the patient.
  • Completing all required documentation timely and accurately.
  • Maintaining a safe, therapeutic environment for patients.
  • Reporting and reviewing changes in patient condition with the registered therapist.
  • Working effectively with patients, families, co-workers, and community resources.
  • Educating patients and families according to the care plan.
  • Ensuring coordination of services and participating in IDT meetings.
  • Adhering to policies regarding schedule changes, documentation, and communication of issues.
  • Understanding and reporting concerns related to corporate compliance to the appropriate authorities.
  • Ensuring compliance with regulatory agencies and organizational policies.
  • Practicing confidentiality in line with HIPAA/HITECH guidelines.
