Job summary

Priority Care Rehab Staffing in Yonkers, NY is seeking a Physical Therapist Assistant (PTA) to support our PT team in a post-acute and long-term care setting. You will work under licensed PT supervision, implementing plans of care and advancing residents’ mobility goals in a SNF environment as an independent contractor with a competitive hourly rate and opportunities for overtime.

This role emphasizes hands-on therapy, accurate documentation, and clear communication with the supervising PT.

Qualifications

• Active New York PTA license is required.
• Current CPR/BLS certification.
• Prior SNF experience preferred.
• Excellent communication and teamwork.

Responsibilities

• Implement PT Plans of Care under the direction and supervision of a licensed Physical Therapist
• Provide therapeutic exercise, gait training, balance programs, and functional mobility treatment
• Document daily treatment notes accurately and in compliance with Medicare, Medicaid, and facility standards
• Communicate patient progress and changes in condition to the supervising PT promptly
• Participate in IDT meetings and support discharge planning as directed by the supervising PT
