The Oaks Nursing Home in New Bedford, MA is seeking a dedicated Physical Therapist to join our care team. You will assess patients, develop individualized treatment plans, and provide therapies across inpatient, outpatient, home health and community settings.

You will collaborate with physicians and nurses, document in EHR, and support discharge planning while educating patients and families. A Massachusetts PT license is required; pay is up to $60/hour with a $4,000 sign-on bonus; workdays

Qualifications

• Active Massachusetts Physical Therapist license or eligibility.
• Experience in inpatient or outpatient settings.
• Strong knowledge of anatomy, physiology, and therapeutic techniques.

Responsibilities

• Assess patients and develop individualized treatment plans.
• Deliver interventions across orthopedics, neurology, geriatrics, pediatrics as needed.

We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a comprehensive understanding of physiotherapy practices, with experience in developing and implementing personalized treatment plans to enhance patient recovery and mobility. As a vital member of our clinical staff, you will collaborate with multidisciplinary healthcare professionals to deliver high-quality patient care across various settings, including inpatient, outpatient, home health, and community care environments.

Duties

• Assess patients' physical conditions through detailed evaluations, including physiology and anatomy knowledge, to determine appropriate treatment strategies.
• Develop and implement individualized care plans aimed at improving mobility, reducing pain, and promoting overall well-being.
• Perform therapeutic interventions such as trigger point therapy, orthopedics, neurology, pediatrics, and geriatrics to facilitate patient rehabilitation.
• Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure cohesive patient management and discharge planning.
• Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) management protocols.
• Educate patients and their families on treatment procedures, home exercises, and community resources to support ongoing recovery.
• Monitor patient progress regularly, adjusting treatment plans as necessary to meet evolving clinical needs while adhering to clinical documentation standards.
• Participate in interdisciplinary team meetings to coordinate care plans for inpatient, outpatient, hospital, or home health settings.

Requirements

• Valid licensure or certification as a licensed Physical Therapist with practicing in a physical therapy role.
• Demonstrated experience in acute care, outpatient clinics, inpatient hospital settings, or outpatient rehabilitation centers.
• Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy.
• Proven ability to develop effective treatment plans tailored to diverse patient populations such as geriatrics, pediatrics, orthopedics, and neurology cases.
• Experience working with electronic health records (EHR) systems or electronic medical records (EMR) systems for documentation purposes.
• Excellent client communication skills with the ability to explain complex procedures clearly and compassionately.
• Familiarity with discharge planning processes and working within home & community care environments is preferred.
• Prior experience in outpatient rehabilitation centers or outpatient clinics is highly desirable.
