Job summary

The Oaks Nursing Home in New Bedford, MA is seeking a dedicated Physical Therapist to join our care team. You will assess patients, develop individualized treatment plans, and provide therapies across inpatient, outpatient, home health and community settings.

You will collaborate with physicians and nurses, document in EHR, and support discharge planning while educating patients and families. A Massachusetts PT license is required; pay is up to $60/hour with a $4,000 sign-on bonus; workdays

Qualifications

Active Massachusetts Physical Therapist license or eligibility.
Experience in inpatient or outpatient settings.
Strong knowledge of anatomy, physiology, and therapeutic techniques.

Responsibilities

Assess patients and develop individualized treatment plans.
Deliver interventions across orthopedics, neurology, geriatrics, pediatrics as needed.
Collaborate with physicians, nurses, and other healthcare professionals for cohesive patient care.
Maintain accurate clinical documentation in EHR/EMR.
Educate patients and families on treatment procedures and home exercises.
Monitor patient progress and adjust plans as needed.
Participate in interdisciplinary team meetings to coordinate care.
