Job summary

Shore Pointe Care Center is seeking a dedicated Full-Time Physical Therapist to join our rehabilitation team in a Skilled Nursing Facility in New Jersey. You will evaluate and treat residents with orthopedic, neurological, and post-surgical conditions to improve mobility and independence.

New grads welcome; mentorship provided. The role requires collaboration with physicians, nurses, and other therapists, documentation in EMR, and participation in care plan meetings to enhance resident quality

Qualifications

• Active Physical Therapist license in NJ or eligible to obtain.
• New grads welcome; mentorship provided.
• Experience in SNF/geriatrics is a plus.

Responsibilities

• Evaluate and treat residents in subacute and long-term care with orthopedic, neurological, and post-surgical conditions.
• Develop personalized plans to improve strength, balance, gait and functional mobility.
• Collaborate with physicians, nurses, and other therapists; document progress in EMR.
• Participate in care plan meetings and family conferences.

About the Role

We are seeking a passionate and dedicated Full-Time Physical Therapist to join our dynamic rehabilitation team in our skilled nursing facility in New Jersey. If you love helping seniors regain their mobility, independence, and quality of life, this is the perfect opportunity for you!

What You’ll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We’re Looking For

• Active Physical Therapist license in the state of New Jersey (or eligible to obtain)
• New grads welcome! We provide excellent mentorship and training
• Previous experience in SNF, long-term care, or geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team
