Job summary

AdvisaCare in Michigan seeks a licensed Physical Therapist to provide patient-centered home health services, developing plans, and helping clients regain mobility in their community. You will work one-on-one with clients, educate families, document progress, and collaborate with an interdisciplinary team.

A valid Michigan license and driver’s license are required; home health experience is preferred though new talent welcomed.

Qualifications

• Licensed Physical Therapist with Michigan license.
• Minimum 1 year home health care preferred; open to new talent.
• Strong communication skills and ability to engage with patients and families.
• Compassionate, patient-centered care mindset.
• Self-starter who can manage schedule and track progress.
• Valid driver’s license and reliable vehicle for travel to patient locations.

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards.
