AdvisaCare is seeking a licensed Physical Therapist in Michigan to join our home health team. You will work with clients in their homes, developing personalized treatment plans and collaborating with interdisciplinary teams to improve mobility and independence.

Qualifications

• Licensed Physical Therapist in Michigan.
• Minimum 1 year home health care experience preferred.
• Strong communication with patients and families.
• Autonomous and punctual with reliable transportation.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility, strength, balance, and function.
• Lead therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, adjusting plans as needed.
• Collaborate with interdisciplinary care teams to ensure continuity of care and outcomes.
• Educate patients, families, and caregivers on exercises and safety techniques.
• Maintain accurate documentation and comply with legal and professional standards.

Requirements

What We're Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan


• Experience - while a minimum of one year in home health care is preferred, we're open to fresh perspectives and new talent!
