Job summary

AdvisaCare is hiring Home Health Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) in Michigan for one-on-one, in-home care. You will help patients regain strength, improve mobility, reduce pain, and safely return to activities they love.

Qualifications

• Degree in Physical Therapy or Physical Therapist Assistant program.
• Current Michigan PT/PTA license in good standing.
• Home health experience preferred.
• Excellent communication and patient education skills.

Responsibilities

• Conduct comprehensive PT evaluations and develop individualized treatment plans.
• Deliver skilled PT services per physician orders and CMS CoPs.
• Maintain timely documentation meeting CMS, payer, and agency standards.
• Collaborate with patients, caregivers, physicians, and the care team to improve mobility and prevent falls.
• Educate patients on exercises, gait training, and home exercise programs (HEPs).
• Reassess progress and modify treatment plans as appropriate.

You Might Be a Great Fit If You:

• Conduct comprehensive physical therapy evaluations to assess functional needs, develop individualized treatment plans, provide evidence-based interventions, and monitor progress to maximize mobility, safety, and independence
• Deliver skilled physical therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs)
• Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards
• Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize mobility, functional outcomes, and fall prevention
• Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention, assistive devices, and individualized Home Exercise Programs (HEPs) to promote safe mobility and recovery
• Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team

Requirements

• Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program
• Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing
• Home health experience preferred (minimum one year)
• Strong clinical judgment, critical thinking, and problem-solving skills
• Excellent communication, interpersonal, and patient education skills
• Ability to work independently while collaborating effectively with an interdisciplinary team
• Reliable transportation, a valid driver's license, and current auto insurance
