Job summary

AdvisaCare in Michigan is seeking a Licensed Physical Therapist to join our community-based home health team. You will develop personalized treatment plans, work one-on-one with clients, and document progress to improve mobility and independence.

We value compassionate, patient-centered care, strong communication, and the ability to manage your own schedule while traveling to patient locations. A valid MI license and reliable transportation are required.

Qualifications

• Licensed Physical Therapist license in Michigan
• Experience in home health care preferred
• Strong communication with patients and families
• Autonomous scheduling and travel to patient locations

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
• Assess and document patient progress, modifying treatment plans as needed
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards

What We're Looking For:
