Job summary

AdvisaCare is seeking a Full-Time Home Health Physical Therapist to provide skilled services across Southeast Michigan, covering Oakland, Macomb, Wayne and Washtenaw Counties.

You’ll perform evaluations, develop plans of care, and deliver individualized therapy in patients’ homes, with a strong emphasis on safety, independence and functional mobility.

Qualifications

  • Degree from an accredited Physical Therapy program.
  • Michigan Physical Therapist license in good standing.
  • Strong clinical assessment, critical-thinking and problem-solving skills.
  • Excellent patient and caregiver education skills.
  • Ability to work independently in the field while collaborating with the interdisciplinary team.
  • Reliable transportation, valid driver’s license and automobile insurance.
  • Home health experience preferred.
  • Familiarity with Medicare/CMS home health documentation requirements preferred.

Responsibilities

  • Complete comprehensive PT evaluations and identify functional limitations and rehabilitation needs.
  • Develop individualized plans of care, measurable goals, and treatment strategies.
  • Provide skilled therapeutic exercise, strengthening, gait, balance and functional mobility interventions.
  • Assess fall risk, home safety, and use of assistive devices.
  • Educate patients and caregivers on safety, mobility techniques and Home Exercise Programs (HEPs).
  • Monitor patient response and progress throughout the episode of care.
  • Complete reassessments and modify treatment plans when appropriate.
  • Coordinate care with physicians, caregivers, nursing and therapy disciplines.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Deliver services per physician orders, established plan of care and CMS requirements.

FULL-TIME HOME HEALTH PHYSICAL THERAPIST WANTED - SOUTHEAST MICHIGAN

We are seeking a compassionate, motivated Full-Time Physical Therapist (PT) to provide skilled home health therapy services throughout the Southeast Michigan area, including communities in Oakland, Macomb, Wayne and Washtenaw Counties.

What You’ll Do

As a Home Health Physical Therapist, you’ll work directly with patients in their homes to improve strength, mobility, balance, safety, and overall functional independence.

  • Complete comprehensive physical therapy evaluations and identify functional limitations and rehabilitation needs.
  • Develop individualized plans of care, measurable goals, and appropriate treatment strategies.
  • Provide skilled therapeutic exercise, strengthening, gait training, balance training, transfer training, and functional mobility interventions.
  • Assess fall risk, home safety, and appropriate use of assistive devices.
  • Educate patients and caregivers on safety, fall prevention, mobility techniques, and Home Exercise Programs (HEPs).
  • Monitor patient response and progress throughout the episode of care.
  • Complete reassessments and modify treatment plans when clinically appropriate.
  • Coordinate care with physicians, caregivers, nursing, therapy disciplines, and other members of the interdisciplinary team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Provide services consistent with physician orders, the established plan of care, and applicable Medicare/CMS requirements.

  • Degree from an accredited Physical Therapy program.
  • Current Michigan Physical Therapist license in good standing.
